Contract Terms & Conditions

All services purchased from Fleet Sports Therapy must be preceded by the applicant acknowledging, by way of completing a PAR-q form (personal training or sports therapy session), intake form (pain clinic) that he/she has no physical limitations preventing him/her from participation or reasons why they will not be able to receive a treatment. All individuals under the age of 18 years must have consent, in written form, from a parent or guardian. The applicant also agrees to abide by the rules and regulations of Fleet Sports Therapy and acknowledges that these rules and regulations may change from time to time. Furthermore, all applicants release Fleet Sports Therapy, its owners, directors, managers and all employees of all liability regarding 1) any injury to self or child 2) any loss or damage to personal property. If purchasing packages that require payments to be made in installments, the applicant agrees to approve such installments be made by credit card on the dates as predetermined in the services contract. All services will either be paid for in advance or at the time of treatment. All packages are to be purchased in advance, and may not be exchanged for cash or transferred to another individual. All packages and offers may not be issued in conjunction with other offers.

Refund & Cancellation Policy

We understand that sometimes a last minute change means that you can no longer make your appointment and we ask that you give us 24 hours notice as we are busy and this allows us to offer the appointment to someone waiting.

Our cancellation policy is as follows :-

​

  • If you cancel your appointment more than 24 hours before treatment then no charge is made.​

  • Less than 24 hours we reserve the right to charge the full amount of the treatment cost

  • No show appointments we reserve the right to charge in full

All cancellation charges are at our discretion to the circumstances


Cancellation – 24 hours notice is required for cancellation of single sessions. Missed sessions without the appropriate notice will be charged as if attended. If you are late for an appointment, this time will be deducted from your session.If you are more than 20min late for your appointment (without communication with your therapist), the session will be considered a 'no show', and you will be charged the full amount for the session.


Medical Cancellations – Cancellation is permitted where an individual is not able to participate in a single sessions for medical reasons. A doctor’s note confirming that the individual was not able to attend the session is required within 7 days of missed session and upon receipt, a credit for a makeup session will be issued.

Gift Cards


Expiration: Gift cards are valid for 6 months from the date of purchase. After the expiration date, the gift card will be void and cannot be redeemed.
Non-Transferable: Gift cards are non-transferable and must be used by the person whose name appears on the card.
No Cash Value: Gift cards cannot be exchanged for cash or refunded. Any remaining balance on a gift card after use will stay on the card until it is used or expires.

 

Payment
Methods: We accept payments by credit/debit card, cash, or approved digital payment methods. Full payment is required at the time of service.
Refunds: Refunds for services are not provided. If you are dissatisfied with a service, please contact us to discuss your concerns.

 

Late Arrivals
Policy: If you arrive late for your appointment, your session may be shortened to ensure the therapist's next appointment starts on time. The full service fee will still apply.

 

Health and Safety
Client Responsibility: Clients are responsible for informing their therapist of any pre-existing conditions, injuries, or health issues prior to treatment. This ensures the therapy provided is safe and effective.
Hygiene: Please maintain good personal hygiene and arrive in clean, comfortable attire suitable for therapy sessions.

 

Liability
Waiver: By booking an appointment, clients agree to release and hold harmless the therapist and business from any liability, including but not limited to, any injuries or conditions that may arise from the therapy provided.
Insurance: It is recommended that clients have personal health insurance that covers therapy services, although it is not mandatory.

 

Privacy
Confidentiality: All client information and treatment records are kept confidential and will not be shared with third parties without the client's explicit consent, except where required by law.
Data Protection: We comply with applicable data protection laws to ensure that your personal information is secure.

 

Right to Refuse Service
Discretion: We reserve the right to refuse service to anyone for any reason, including but not limited to inappropriate behavior, health concerns, or non-compliance with our policies.

 

Amendments
Policy Changes: These terms and conditions are subject to change without notice. Please review this page regularly for updates. Your continued use of our services constitutes acceptance of the latest terms and conditions.

Cancellations

If you need to cancel your appointment, please contact us via email at lh@fleetsportstherapy.co.uk or call/text 07525049144. Cancellations must be confirmed at least 24 hours before your scheduled appointment. Failure to do so will result in a late cancellation fee.
